1. Who we are

Healthy Life by Dr. Amelinckx LLC (“HealthyLife”) offers telehealth visits and weight-loss programs to adults located in Florida. This policy explains what data we use when you visit healthylifeus.com, fill out a form, book a visit or buy.

2. What data we collect

• Contact: name, email, phone, date of birth and address.
• Health: weight, height, goals, medical conditions, medications and anything you write in the forms or during the visit.
• Visits and purchases: appointment date, products, payments and shipping. Stripe processes payments; we do not store your full card number.
• Technical data: IP address, browser and pages visited. See the Cookie Policy.

3. How we use it

• To evaluate and care for you and, if the doctor recommends it, coordinate your treatment and its shipping.
• To book your visit and send you reminders.
• To charge, comply with the law and protect the site.
• To send you tips and news, if you agreed to it in the form. You can unsubscribe anytime.

Your health data is never used for advertising.

4. Who we share it with

We do not sell your data. We only share it with:

• Dr. Adriana Amelinckx and the clinical team: your evaluation and follow-up.
• SimplePractice: storing your medical record in a HIPAA-compliant system.
• Licensed pharmacy: preparing and shipping your medication, only if the doctor prescribes it.
• Stripe: processing payments.
• Google (Calendar and Meet): booking and holding the video visit.
• WebGo: the platform for the website and forms.
• Google Analytics and Meta: measuring visits and ads. They may receive that you submitted a form and your contact details; never your health answers.
• Email, SMS and WhatsApp providers: sending you the messages you agreed to.
• Authorities: only when the law requires it.

These providers may only use your data to provide their service to us.

5. How long we keep it

We keep your medical record for as long as Florida law requires (at least 5 years from your last contact). We keep contact and marketing data while you are a customer or until you ask us to delete it.

6. How we protect it

We use encrypted connections, password-protected access and permissions only for those who need them. No system is 100 % secure. If a breach affects you, we will notify you as Florida law requires.

7. Your rights

You can ask to see, correct or delete your data, get a copy of your record and stop receiving messages. Write to us at contact@healthylifeus.com. Some medical data cannot be deleted before the legal period ends. To stop receiving messages you can also use “unsubscribe” in the email or reply STOP to an SMS.

8. Minors

We only serve people 18 and older and do not knowingly collect data from minors.

9. Changes

If we change this policy, we will post the new date at the top. If the change is important, we will notify you by email.
