1. Acceptance
By using healthylifeus.com you accept these terms. If you do not agree, do not use the site.
2. Florida only
Healthy Life cares only for people who are physically in Florida at the time of the consultation. We may refuse service if you are outside Florida.
3. Age and truthful information
You must be at least 18 years old. You confirm that what you submit is true.
4. What we offer
Healthy Life offers weight-loss support, which may include a telehealth consultation. Medical care is provided by a licensed professional. We do not promise a weight or health result.
5. Accepting telehealth by submitting the form
By submitting a form on this site you accept these telehealth terms: the visit may be virtual and has limits compared with an in-person visit; you must be in Florida during the consultation; this does not create a provider-patient relationship until a professional evaluates you; and, in an emergency, you must call 911.
6. Email and WhatsApp
If you check the form box, you authorize us to message you by email and WhatsApp to answer your request and send updates. You can ask us to stop at contact@healthylifeus.com.
7. Payment and cancellation
Payments are processed by Stripe. Prices may change. You may cancel a plan before the next renewal. A consultation already provided is not refunded, unless the law requires it.
8. Your conduct
You must not lie about your identity or location, request medication or services fraudulently, or use the site for anything illegal.
9. Liability
To the extent the law allows, Healthy Life is not liable for indirect damages. Total liability will not exceed what you paid for the service.
10. Governing law
These terms are governed by the laws of the State of Florida, United States.
